Faxes are handled in the same time frame as regular mail. Emails cannot be accepted for transcript requests, unless they include an attached PDF of a completed/signed transcript request form.
VERIFICATION OF MEDICAL DEGREE/CERTIFICATION LETTERS
- There is NO fee for this service.
- Certification letter requests can be faxed or mailed with a SIGNED and DATED release form.
VERIFICATION OF RESIDENCY PROGRAM COMPLETION

DENTAL VERIFICATION LETTERS
- Dental school verification letters for D.D.S. graduates are handled by the Water Tower Campus Registration & Records Office. Verification request information can be obtained by calling: (312) 915-7221.
