During a student’s time at Stritch there will be several forms that will need to be complete. This page was created to assist you in finding those forms with ease. All the forms are in myLumen which require students to login. If you do not find a form that is required of you to complete, please feel free to contact the Office of Student Affairs at (708) 216-3220 and we will be glad to further assist you.
